YOUTHLINK’s family support program serves youth with intellectual disabilities and their families.
Eligibility:Youth aged 12-21 years who have a diagnosis of intellectual disability and who are living at home with their families.
Area Served: Our Family Support Program provides services all over Toronto.
Access: Our office is wheel chair accessible. Family Support Workers also do home visits, which may be more appropriate for some families and youth, for whom travel is difficult.
Services Provided: Family Support services are based on case management model. This is a joint process where adolescents and their families meet with a family support worker to develop a plan based on their identified needs. The collaborative process also involves working closely with doctors, psychiatrists, psychologists, behaviour therapists, schools, various funding sources and respite services just to name a few. Supportive counselling may be offered to help deal with the issues of living with and/or supporting some one with an intellectual disability.
We also offer a monthly parent support group for parents of young people aged 12-21 who have an intellectual disability. These groups provide a forum for information sharing, emotional support and education regarding different disabilities through professional resources and information sharing.
Hours of Service: Monday to Friday from 9am -5pm. Also, evening appointments can be scheduled, when necessary.
