DESCRIPTION:

This position is responsible for providing culturally responsive intensive in-home supports for youth 11-15 years old who are either at immediate risk of entry into care or have entered care within the last 7 to 30 days. The goal is to have youth remain at home with their caregivers or for youth who have entered care, to return them to their caregiver in a timely fashion. The position is responsible for identifying and coordinating crisis plans, identifying supports, strengths and assets, providing ongoing interactions to build rapport and enhance guidance, providing advocacy, referrals and facilitating the specific needs of the family. This position is responsible for providing brokerage and advocacy and entails working collaboratively with youth, caregivers and community service providers.

Key Responsibilities:

  • Processes referrals with 24 hours or within the same day if there is a crisis. Contact the referring CAS worker to clarify case information and goals

  • Provides service to youth and their families when the youth are likely to enter care or within a week of their entrance into care

  • Partners with the child welfare team in supporting hard to engage youth and their families who are experiencing significant conflict and risk

  • Service will be accessible in meeting the needs of the youth and family, hours will be flexible

  • Develops a customized intervention plan in conjunction with the youth, family and child welfare team that is informed by equity, critical reflective practice and trauma informed practice. The plan must be youth-led, have clear goals and expectations and identify needed supports and resources,

  • Frequent contact with the youth where appropriate will be used to build a positive trusting relationship and effect behavior change. This includes accompanying youth to appointments i.e. CAS meetings, counselling services, court proceedings etc.

  • Advocates for the youth and connects them and their family to needed resources and services. Assist the youth to develop positive relationships with other professionals where appropriate

  • Completes client and administrative reports, case notes, and statistics in accordance with organizational guidelines. Regularly collect and input data as determined by CAST and the Journey to Zero Program

  • Assesses need for crisis/safety plan and implements immediately if needed.

  • Participates in individual supervision, team meetings, child welfare meetings, and has a weekly presence at various CAS site

  • Creates an end of service plan which includes relevant referrals and resources

  • Actively contributes to the Agency’s Strategic Plan by participating in initiatives and committees to support the overall Plan.

  • Takes all reasonable and necessary precautions to protect his or her own health and safety and that of co-workers by complying and demonstrating knowledge of the policies, procedures and safe practices established by YouthLink.

 

REQUIRED QUALIFICATIONS

EDUCATION:

  • Diploma in Child & Youth Worker/Undergraduate degree in social work, psychology or related degree

  • Specialized training in de-escalation, conflict resolution, mediation and negotiation skills an asset

  • Knowledge of equity and Anti-Black Racism practice frameworks

EXPERIENCE:

  • Two (2) years’ experience working with high need youth and families using a strength-based approach

  • Demonstrated experience with youth engagement and working successfully with community partners.

  • Demonstrated ability and experience in working with marginalized youth, their families, newcomers and diverse youth populations living in under-served communities.

REQUIRED SKILLS:

  • Strong interpersonal and communication skills.

  • Operating using an Anti-Black Racism lens.

  • Strong youth and family engagement skills.

  • Demonstrated ability to establish effective working relationships with community service partners.

  • Strong communication and interpersonal skills.

HOURS OF WORK:

  • 35 hours per week

  • Requires evening hours of up to 2 evenings per week and occasional weekend hours

  • Travelling required

SALARY:

$ 45,613– 51,675
