REPORTS TO:

Manager, Youth Transitional Housing and Shelter Program

DESCRIPTION:

This position works with the case management team to support and provide specialized Follow-up Support Services to youth at risk of homelessness. They will act as a transition worker for youth seeking independent living options in the community.

 

Key Responsibilities:

 

  • Assist front line staff in the development of consistent case plans, group activities, mediation services and transition of youth from the shelter into long-term accommodation.

  • Facilitate housing related programming to clients

  • Incorporate into the work a strengths-based relationship-based philosophy of care and a belief in the resilience of youth.

  • Promote a safe and therapeutic environment and establish therapeutic relationships with clients through role modeling consistent with our agency mission, vision and values.

  • Assess and understand client mental health needs through a developmental and trauma informed lens and formulate methods of intervention.

  • Support clients in sustaining housing who have obtained secured accommodations.

  • Assist clients with securing appropriate housing by developing an appropriate housing plan which could include market rent apartments, co-ops or group homes that are in accordance with Hostel Services guidelines.

  • Assist with connecting clients to medical services.

  • Support clients to integrate into their community.

  • Provide pre-housing support services, including, but not limited to:

  • Assist youth in organizing and applying for community start up allowance from the welfare office.

  • Secure identification.

  • Provide independent living skills training and education on tenant rights and responsibilities.

  • Provide after care support for vulnerable youth through CSS drop-in and home visiting.

  • Promote and integrate youth and family engagement.

  • Develop appropriate discharge plan in accordance with Hostel Services guidelines.

  • Provide individual and group workshops geared to independent living skills.

  • Support youth during negotiations with landlords. Landlord outreach and conflict resolution once placed.

  • Liaise with furniture bank and other services geared toward furnishing their new accommodations.

  • Assist youth in selecting an appropriate roommate for shared accommodation.

  • Assist youth in developing and following a housing budget.

  • Ensure that client records are maintained as per agency standards and in accordance with privacy policies and legislation.

  • Ensure that administrative duties are completed within required timeframes including completing a housing history and assessment.

  • Ensure accurate data collection and report writing

  • Effectively utilize communication systems to convey client and program related information.

  • Represent the Agency in a professional manner when dealing with families, external professionals, general public and community partners.

  • Work in accordance with all policies and procedures of the Agency.

  • Promote an environment of inclusion that promotes equity and respects diversity.

  • Perform other duties as required.

  • Actively contribute to the Agency’s Strategic Plan by participating in initiatives and committees to support the overall Plan.

  • Take all reasonable and necessary precautions to protect his or her own health and safety and that of co-workers by complying and demonstrating knowledge of the policies, procedures and safe practices established by YouthLink.

 

REQUIRED QUALIFICATIONS:

EDUCATION:

  • Undergraduate Degree in Social Work or Degree / Diploma in Child and Youth Worker or a combination of directly related education and experience.

  • Minimum of 2 years’ experience as a front-line worker, preferably in a residential, housing, outreach, or shelter setting.

  • Valid First Aid / CPR certification.

  • Valid CPI certification is an asset.

  • Completion of required suicide prevention and intervention training.

EXPERIENCE / SKILLS

  • 2 years’ experience in the shelter system, or housing, or a residential setting.

  • Demonstrated commitment to inclusiveness, diversity and anti-oppressive practices.

  • Operating using an Anti-Black Racism lens is an asset.

  • Demonstrated leadership skills.

  • Knowledge of adolescent developmental theory and specific issues related to street youth.

  • Housing and community resources experience an asset.

  • Working knowledge of the Tenant Protection Act and the welfare system.

  • Demonstrated knowledge and experience with at risk adolescents and young adults.

  • Understanding of the complex issues facing our clients.

  • Appreciation of the importance of engaging youth and families from a strengths-based relationship-based approach.

  • Sensitivity to and ability to work with a diverse client and staff group

  • Sound knowledge of the issues facing homeless and street-involved youth.

  • Sound knowledge of the City of Toronto Hostel Standards and all other applicable legislation (CFSA, YCJA, Mental Health Act, etc.).

  • Strong professional network and knowledge of relevant community resources.

  • Ability to exercise professionalism and sound judgement in conflictual and stressful situations.

  • Ability to work independently and effectively as part of a multi-disciplinary team.

  • Excellent interpersonal and conflict resolution skills. Ability to resolve issues with supervisors and co-workers in a manner that is respectful and cooperative in accordance with the agency culture and philosophy.

  • Flexible team player with strong administrative, organizational skills, time management skills, the ability to multi-task, shift priorities, handle interruptions and meet deadlines.

  • Strong written and verbal communication skills. Ability to communicate in both official languages and/or other languages is a definite asset.

  • Ability and discretion to deal tactfully with different clientele who can at times be difficult / aggressive.

  • Demonstrated proficiency in the range of Microsoft Office products (intermediate Word, Outlook, Excel, PowerPoint) and internet mandatory. Proficient and accurate keyboard skills.

  • Ability and discretion to maintain delicate and confidential information.

  • Attention to details and high level of accuracy.

 

ADDITIONAL

REQUIREMENTS:

  • Valid ‘G’ class licence with 3 years’ clean abstract

  • Offsite travel is required

  • Police Vulnerable Sector Check required.

  • Medical clearance including up to date vaccination and tuberculosis testing.

WORK HOURS:

35 hours per week (including two evenings per week and one Saturday per month to accommodate client traffic)

SALARY:

$44, 939 – $50, 911 per annum

Our E-Counselling What’s Up Walk-In service will be open Monday – Friday until further notice. If you would be interested in booking one of our daily video-conferencing appointments for a counselling or support session with a clinician, call 416-967-1773 extension 222 and you will be connected to our Intake department. E-Counselling and support is also available for many of our other programs. Intake staff can provide you with more information if you are interested.

 

Although many of our staff are temporarily working off-site, all of our housing and residential programs continue to operate as usual.
