REPORTS TO: Senior Manager, Youth Transitional Housing and Shelter Program



This position works with the case manager to support and provide specialized Follow-up Support Services to youth at risk of homelessness. They will act as a transition worker for youth seeking independent living options in the community.


  • Assist front line staff in the development of consistent case plans, group activities, mediation services and transition of youth from the shelter into long-term accommodation.
  • Incorporate into the work a strengths-based relationship-based philosophy of care and a belief in the resilience of youth.
  • Promote a safe and therapeutic environment and establish therapeutic relationships with clients through role modeling consistent with our agency mission, vision and values.
  • Assess and understand client mental health needs through a developmental and trauma informed lens and formulate methods of intervention.
  • Support clients in sustaining housing who have obtained secured accommodations.
  • Assist clients with securing appropriate housing by developing an appropriate housing plan which could include market rent apartments, co-ops or group homes that are in accordance with Hostel Services guidelines.

    • Provide after care support for vulnerable youth through CSS drop-in and home visitin
    • Promote and integrate youth and family engagemen
    • Developing an appropriate discharge plan in accordance with Hostel Services guideline
    • Individual and group workshops geared to independent living skill
    • Supporting youth during negotiations with landlords. Landlord outreach and conflict resolution once place
    • Liaison with furniture bank and other services geared toward furnishing their new accommodation
    • Assisting youth in selecting an appropriate roommate for shared accommodatio
    • Assisting youth in developing and following a housing budget.
  • Ensure that client records are maintained as per agency standards and in accordance with privacy policies and legislation.
  • Ensure that administrative duties are completed within required timeframes including completing a housing history and assessment.
  • Effectively utilize communication systems to convey client and program related information.
  • Represent the Agency in a professional manner when dealing with families, external professionals, general public and community partners.
  • Work in accordance with all policies and procedures of the Agency.
  • Promote an environment of inclusion that promotes equity and respects diversity.Ensure that client records are maintained as per agency standards and in accordance with privacy policies and legislation.

  • Actively contributes to the Agency’s Strategic Plan by participating in initiatives and committees to support the overall Plan.
  • Takes all reasonable and necessary precautions to protect his or her own health and safety and that of co-workers by complying and demonstrating knowledge of the policies, procedures and safe practices established by YouthLink.




  • Undergraduate Degree in Social Work or Degree / Diploma in Child and Youth Worker or a combination of directly related education and experience.
  • Minimum of 2 years’ experience as a front-line worker, preferably in a residential, housing, outreach, or shelter setting.
  • Valid First Aid / CPR certification.
  • Valid UMAB certification.


  • 2 years’ experience in the shelter system, or housing, or a residential setting.
  • Demonstrates a commitment to inclusiveness, diversity and anti-oppressive practices.
  • Demonstrated leadership skills.
  • Knowledge of adolescent developmental theory and specific issues related to street youth.
  • Preference will be given to candidates who have housing and community resources experience.
  • Working knowledge of the Tenant Protection Act, and the welfare system.
  • Demonstrated knowledge and experience with at risk adolescents and young adults.
  • Understanding of the complex issues facing our clients.
  • Appreciation of the importance of engaging youth and families from a strengths-based relationship-based approach.
  • Sensitivity to and ability to work with a diverse client and staff group
  • Sound knowledge of the issues facing homeless and street-involved youth.
  • Sound knowledge of the City of Toronto Hostel Standards and all other applicable legislation (CFSA, YCJA, Mental Health Act, etc.).
  • Strong professional network and knowledge of relevant community resources.
  • Ability to exercise professionalism and sound judgement in conflictual and stressful situations.
  • Ability to work independently and effectively as part of a multi-disciplinary team.
  • Ability to resolve issues with supervisors and co-workers in a manner that is respectful and cooperative in accordance with the agency culture and philosophy.


  • Excellent interpersonal and conflict resolution skills.
  • Strong administrative and organizational skills.
  • Completion of required suicide prevention and intervention training.
  • Flexible person, team player who has strong organizational skills, time management skills, the ability to multi-task, shift priorities, handle interruptions and meet deadlines.
  • Strong written and verbal communication skills. Ability to communicate in both official languages and/or other languages is a definite asset.
  • Ability and discretion to deal tactfully with different clientele who can at times be difficult / aggressive.
  • Demonstrated proficiency in the range of Microsoft Office products (intermediate Word, Outlook, Excel, PowerPoint) and internet mandatory.       Proficient and accurate keyboard skills.
  • Ability and discretion to maintain sometimes delicate and confidential information.
  • Attention to details and high level of accuracy
  • This position requires irregular daytime and evening hours to accommodate client traffic.
  • Police Vulnerable Sector Check required.
  • Medical clearance including up to date vaccination and tuberculosis testing.

HOURS OF WORK: 21 hours per week. 

SALARY: $44,939 to 50,911 (to be pro-rated based on hours worked)
