REPORTS TO:Senior Manager, Clinical Services
DESCRIPTION:
This position is responsible for providing Wraparound Servicesfor youth 12-16 years’ old who are referred by Child Welfaredue to being at risk of going into care or who are currently incare and require support to return home.This position is also responsible for guiding the family througha process of Wraparound stages, i.e. identifying andcoordinating crisis/safety plans, identifying supports to build aWraparound team, identifying family goals, providing ongoingguidance and coordination of services, advocacy, referrals andfacilitating the specific needs of the family.
Key Responsibilities: 

Facilitates youth/family home meetings to address crisis, assess strengths, needs, culture, values, beliefs, and coping strategies. Identifies life domain areas to support the family’s vision/goals. Liaises, advocates, coordinates, mediates meetings with external resources i.e. schools, counselling, health and parental supports, etc. 
Collaborates intensively and actively with child welfare, often at child welfare offices, including spending significant time in briefing, updating, liaising and engaging child welfare staff and management and conducting client and administrative tasks at child welfare offices. Completes client and administrative reports, case notes, and statistics in accordance with organizational guidelines. Responds to referrals, consults with child welfare to determine eligibility.
Promotes and orients youth/family to Wraparound; Assists family in identifying and developing a team of formal/informal supports. Contacts potential team members, explains the process, secures commitment and coordinates meetings.
Assesses need for crisis/safety plan and implements immediately if needed.
Develops, monitors, and revises the Wraparound Plan. Records and distributes plans. Participates in individual supervision, peer support, child welfare meetings, Wraparound Canada, Wraparound Ontario and relevant training opportunities. Actively contributes to the Agency’s Strategic Plan by participating in initiatives and committees to support the overall Plan.
Takes all reasonable and necessary precautions to protect his or her own health and safety and that of co-workers by complying and demonstrating knowledge of the policies, procedures and safe practices established by YouthLink.

REQUIRED QUALIFICATIONS
EDUCATION:Undergraduate Degree in Social Work, Psychology or Degree / Diploma in Child & Youth Worker related field.Specialized training in de-escalation, conflict resolution, mediation and negotiation skills. 
EXPERIENCE:Two (2) years’ experience working with youth and families, parenting groups, strength-based work, team-building and working with high need youth and families. Good understanding of issues faced by diverse populations.Knowledge of Mental Health Diagnosis and potential behaviours exhibited by youthExperience working from an Anti-Black Racism Lens is an asset. Wraparound facilitator training an asset.
REQUIRED SKILLS:Strong youth and family engagement skills.Demonstrated ability to establish effective working relationships with community service partners.Strong communication and interpersonal skills.
ADDITIONAL REQUIREMENTS:The position requires evening hours of two (2) evenings per week. Some local travelling required.
HOURS OF WORK:35 hours per week
SALARY:$ 46,298 – $52,450

We know this is a stressful time for everyone, and we want you to be able to continue to access our counsellors and clinicians in a safe and secure way so we are now offering you online video-conferencing counselling and support.

 

Our E-Counselling What’s Up Walk-In service will be open Monday – Friday until further notice. If you would be interested in booking one of our daily video-conferencing appointments for a counselling or support session with a clinician, call 416-967-1773 extension 222 and you will be connected to our Intake department. E-Counselling and support is also available for many of our other programs. Intake staff can provide you with more information if you are interested.

 

Although many of our staff are temporarily working off-site, all of our housing and residential programs continue to operate as usual.
